The DAISY Award recognises the compassion, dedication and clinical excellence our nurses and midwives show every day. Your nomination is a powerful way to say thank you. 

How to nominate

  • Complete our quick and easy online DAISY nomination form. There are posters around the Trust with QR codes which can be scanned from your phone.

  • Printed forms and a nomination form box are also available from receptions.

  • Download a printable nomination form and drop it at our main hospital reception or post it to us at: DAISY Award Team, West Hertfordshire Teaching Hospitals NHS Trust, Willow House, Watford General Hospital, Vicarage Road, Watford, WD18 0HB. 

The forms are also available in an Easy Read version.

Who can be nominated? 

You can nominate any registered nurse or midwife working at West Herts, including: 

  • Ward nurses and midwives 

  • Specialist nurses and midwives 

  • Any other registered nurse or midwife involved in your care

Why it matters

The DAISY award is a global recognition programme celebrating nurses and midwives who provide exceptional, compassionate care. 

It was created by the family of J. Patrick Barnes, who received exceptional care from nurses before his passing in 1999. The Foundation was established to recognise the compassion and skill nurses bring to patients and families every day. 

Patients, families or carers can nominate a nurse or midwife.

We’re looking for examples of: 

  • Compassionate care

  • Going above and beyond 

  • Clinical excellence 

  • Positive impact on your experience 

Yes. You can include more than one individual in your nomination. If submitting multiple names, please clearly describe each person’s contribution.

Currently, nominations are for individual nurses or midwives only. 

All nominations are judged anonymously by our DAISY Award nomination committee. Winners are selected based on the DAISY awards criteria reflecting the Trust's values: Empowered,  Compassionate, Professional and Inclusive. 

There are twelve winners per year. All nominees are recognised internally with a DAISY pin badge. Each DAISY Award recipient receives:  

  • A certificate with your nomination story 

  • A hand-carved “Healer’s Touch” statue 

  • A public presentation in their workplace 

Yes, nominees are informed and your message is shared with them.
