Sign up for quick and easy online access to your hospital health record, appointment times and easier communication with your care team.

What is the Patient Portal?

The Patient Portal provided by West Herts Teaching Hospitals NHS Trust is a completely secure way to view and manage your own hospital health information.

The Patient Portal is an online system that allows you to look at information held on your hospital record, such as hospital appointments, clinical documentation and test results.

You need to be aged 18 or over to use the Patient Portal which can be accessed from a smartphone or any device with internet access.

The patient information leaflets that you will find on the following pages are about conditions, treatments and services provided by West Hertfordshire Teaching Hospitals NHS Trust.

These leaflets are not meant to replace the information, advice and support provided to you by our staff but they may help you and your relatives and carers answer some questions or help you think about questions that you might ask your doctor or nurse.

They are written by health professionals across the trust and are reviewed regularly to make sure all the information is up-to-date and to ensure the delivery of our vision; the very best care to every patient, every day.

Patient bedside folder

Picture of the My Bedside Folder The patient bedside folder contains everything you need to know if you're coming to one of our hospitals as an inpatient.

It includes information about what to expect while you are on a ward, the staff looking after you, retail outlets in our hospitals, the discharge procedure, how to get to and from our hospitals, how you can give feedback and what additional support is available.

It's well worth having a read before you come into hospital as it lets you know what to bring and what you can expect while you're with us. Relatives of patients may find it useful too.

My Care Record

My Care Record enables health and care professionals to access information about you to improve your care.

West Hertfordshire Teaching Hospitals NHS Trust is part of My Care Record, an approach to improving care by joining up health and care information. Wherever possible, health and care professionals will be able to access your records from other services when it is needed for your care. For example, a hospital doctor, community nurse, or social worker involved in your care could view the information they need from your GP record. More information, including answers to frequently asked questions and a list of the organisations that are taking part can be found on the My Care Record website www.mycarerecord.org.uk


Health and Care Video Library

The Health and Care Video Library includes over 600 simple and easy to follow information videos. The clinically assured content has been produced in partnership with Torbay and South Devon NHS Foundation Trust and covers a range of topics and conditions in an easily accessible format, providing an alternative to written health information.
