Age concern
Age Concern can provide a volunteer to visit an older person in her or his own home following a stay in hospital. The scheme is known as the 'Hospital Discharge Scheme'.
The aim of the scheme is to provide some social contact, support and encouragement when first returning home. By doing this they hope to enable the client to feel more confident in their home environment and surrounding area.
