Sign up for quick and easy online access to your hospital health record, appointment times and easier communication with your care team.
What is the Patient Portal?
The Patient Portal is a completely secure way to view and manage your own hospital health information.
It's an online system that allows you to look at information held on your hospital record, such as hospital appointments, clinical documentation and test results.
You need to be aged 18 or over to use the Patient Portal which can be accessed from a smartphone or any device with internet access.

Do you still need your appointment?
We don’t want you to wait longer than necessary for an appointment so we’re checking to make sure those who are waiting for certain appointments still need them.
There are many reasons why a patient may no longer need their appointment. Their symptoms could have improved or they could have been treated elsewhere. By checking our appointment lists, we can help to reduce the number of patients who do not attend an arranged appointment because they no longer require treatment.
We will send you a text message from ‘WestHerts’ with a link inviting you to log on to our Patient Portal. You’ll then be asked to complete a short questionnaire about your appointment. Please be assured this is a genuine text message and not a scam.

The patient information leaflets that you will find on the following pages are about conditions, treatments and services provided by West Hertfordshire Teaching Hospitals NHS Trust.
These leaflets are not meant to replace the information, advice and support provided to you by our staff but they may help you and your relatives and carers answer some questions or help you think about questions that you might ask your doctor or nurse.
They are written by health professionals across the trust and are reviewed regularly to make sure all the information is up-to-date and to ensure the delivery of our vision; the very best care to every patient, every day.

Digital consent for treatment or a procedure at our hospitals
We have introduced a digital consent system called Concentric to replace paper consent forms. This allows us to work more effectively and provide patients with information that is more personal to their individual care.

My Care Record
My Care Record enables health and care professionals to access information about you to improve your care.
West Hertfordshire Teaching Hospitals NHS Trust is part of My Care Record, an
approach to improving care by joining up health and care information. Wherever
possible, health and care professionals will be able to access your records from
other services when it is needed for your care. For example, a hospital doctor,
community nurse, or social worker involved in your care could view the
information they need from your GP record. More information, including answers
