Information leaflets

The patient information leaflets that you will find on the following pages are about conditions, treatments and services provided by West Hertfordshire Teaching Hospitals NHS Trust.

These leaflets are not meant to replace the information, advice and support provided to you by our staff but they may help you and your relatives and carers answer some questions or help you think about questions that you might ask your doctor or nurse.

They are written by health professionals across the trust and are reviewed regularly to make sure all the information is up-to-date and to ensure the delivery of our vision; the very best care to every patient, every day.

Patient information leaflets produced by the trust and other respected healthcare charities and organisations can be found below. The Trust cannot be held responsible for the accuracy of information contained within leaflets produced by external organisations. Please refer to the original author.

Digital consent for treatment or a procedure at our hospitals

We have introduced a digital consent system called Concentric to replace paper consent forms. This allows us to work more effectively and provide patients with information that is more personal to their individual care.

My Care Record

Picture of a My Care Record poster showiong a smiling woman My Care Record enables health and care professionals to access information about you to improve your care.

West Hertfordshire Teaching Hospitals NHS Trust is part of My Care Record, an approach to improving care by joining up health and care information. Wherever possible, health and care professionals will be able to access your records from other services when it is needed for your care. For example, a hospital doctor, community nurse, or social worker involved in your care could view the information they need from your GP record. More information, including answers to frequently asked questions and a list of the organisations that are taking part can be found on the My Care Record website
