Services offered – on one or more sites:

  • Multidisciplinary  specialist clinics (consultant, specialist dietitian and/or DSN and/or specialist nursing assistant and/or mental health support present)
    • Combined antenatal
    • Foot
    • Pump (CSIII)
    • Review
    • Young Adult (aged 16-25 years)
  • DSN/dietician led clinics with mental health support as needed
  • MDT meetings for foot and mental health
  • Education programmes for patients and staff e.g. Type 1 refresher; pump awareness; GDM; Sugar and Spice
  • Multi Disciplinary Foot Clinics
  • Multi Disciplinary Foot Ward Rounds

Diabetes foot team

Multidisciplinary Diabetes foot clinics – for patients with active foot wounds/ Charcot Neuroarthropathy

  • Patient will have an appointment with the Diabetes consultant and Diabetes specialist podiatrist to review the active foot wound
  • A treatment plan will be discussed with the patient
  • Patients will receive on going care which may be shared with the community podiatry team. either continue to be seen in the MDFT or referred to community podiatry
  • Foot pathways – The process outlined below details how patients with diabetic foot disease should be managed depending on the risk that they are currently presenting
