Services offered – on one or more sites:
- Multidisciplinary specialist clinics (consultant, specialist dietitian and/or DSN and/or specialist nursing assistant and/or mental health support present)
- Combined antenatal
- Foot
- Pump (CSIII)
- Review
- Young Adult (aged 16-25 years)
- DSN/dietician led clinics with mental health support as needed
- MDT meetings for foot and mental health
- Education programmes for patients and staff e.g. Type 1 refresher; pump awareness; GDM; Sugar and Spice
- Multi Disciplinary Foot Clinics
- Multi Disciplinary Foot Ward Rounds
Diabetes foot team
Multidisciplinary Diabetes foot clinics – for patients with active foot wounds/ Charcot Neuroarthropathy
- Patient will have an appointment with the Diabetes consultant and Diabetes specialist podiatrist to review the active foot wound
- A treatment plan will be discussed with the patient
- Patients will receive on going care which may be shared with the community podiatry team. either continue to be seen in the MDFT or referred to community podiatry
